  • Form Completion: A civil surgeon completes and signs Form I-693, a 14-page document, which the applicant submits to USCIS.
  • Must Be Performed In Person: Obviously, the applicant must be physically present for the physical examination.

Vaccination Portion Only (VPO) Service:

  • Limited Eligibility: Only certain applicants undergoing an immigration status adjustment qualify for VPO, for example, many applicants with K-1 visas. It is your (or your legal advisor’s) responsibility to determine if you are eligible for this limited service. The civil surgeon does not determine your eligibility.
  • Remote Process: Applicants can complete the entire vaccination process via email and regular post, which is particularly convenient for those who may have difficulty attending in-person appointments or for those who reside outside of the Kansas City area. This option allows for the necessary paperwork to be handled online, minimizing the need for physical visits.
  • Limited Scope: Focuses solely on verifying and ordering required vaccinations.
  • Vaccination Record Review: Evaluate existing vaccination records to determine any missing vaccines.
  • Partial Form Completion: The civil surgeon completes and signs only the vaccination section of Form I-693 (pages 1, 2, 3, 4, 5, 12, 13, & 14 of the 14-page form).
  • In-Person Pickup: Dr. Gordon offers the option to pick up documents directly from his office for those who prefer to handle their paperwork in person. This is an efficient option for those who reside in the Kansas City area, as it allows for immediate signing and processing.

The Vaccination Portion Only (VPO) service helps immigrants meet U.S. vaccination requirements for their green card applications. Here's how it works:

  • Record Review: Bring (by appointment) or email your existing vaccination records to the civil surgeon for review.
  • Vaccine Administration (if needed): If you need some vaccines to meet USCIS requirements, the civil surgeon will provide you with written orders for the vaccines. You may obtain the vaccines anywhere you like, such as from the local health department, your personal doctor, or your favorite pharmacy.
  • Form Completion: Once everything is complete, the doctor will finalize and sign the vaccination section of Form I-693.
  • Verification of Identity: This may be performed in person by appointment, or remotely via Skype, Zoom, or FaceTime.
  • Sign Your Report: The applicant must sign page two of the report either in person or through the mail.
  • Receive Your Report: You'll be issued a report in a tightly sealed envelope verifying your vaccination status, which you will include with your green card application. You may either pick it up in person, or it will be mailed to you. An ordinary copy of the report will also be issued to you. YOU MUST SUBMIT THE OFFICIAL REPORT WITH THE SEAL INTACT!

In 2024, depending on age and vaccination history, U.S. immigrants require various vaccines, such as COVID-19, measles, mumps, rubella, varicella, hepatitis B, influenza, pneumonia, meningitis, polio, tetanus, diphtheria, and pertussis. Your civil surgeon will determine which vaccinations are required in your particular case.

There is no medical reason why you cannot obtain all needed vaccinations at the same time. This has been scientifically proven to be safe and effective. However, some vaccinators have a policy limiting vaccinations to no more than 3 or 4 at a time, even though there is no medical reason for this.
