I-693 Form: Everything You Should Know About

As per the U.S Citizen and Immigration Services (USCIS), applicants applying for adjustment of immigration status have to undergo a medical examination as evidence that they don’t have any inadmissible illnesses or health conditions. As an integral part of the process, candidates must submit Form I-693, which includes a vaccination record and a medical examination report.

What Is the Purpose of Form I-693?

The purpose of Form I-693 is to summarize the results of the medical examination of an applicant seeking permanent resident status in the USA. The results are presented to the USCIS. With the help of this examination report, USCIS establishes which applicants seeking immigration benefits, are permitted to remain in the country from a medical standpoint.

Important Documents Required for the Immigration Medical Exam

Applicants need to furnish certain documents, some of which include:

  • All available vaccination records, including your COVID-19 vaccination record.
  • A valid, current, government-issued photo identity card such as state ID, travel/work permit, passport, driver’s license, etc.
  • Tuberculosis testing and treatment records if applicable, including chest x-ray reports--do not bring chest x-ray films unless requested.
  • Syphilis testing and treatment records if applicable.
  • Gonorrhea testing and treatment records if applicable.

What If I Receive an I-693 Deficiency Notice?

If you receive an I-693 deficiency notice, do not panic or worry. It does not mean that your I-485 (the application to adjust status or application to register permanent residence) has been rejected. The application will be delayed for review, and you will be given time to produce the medical report prior to or during your interview.

What Is the Right Time to Submit Form I-693?

Previously, it was recommended that you submit Form I-693 with Form I-485. However, the current standing is that you don’t need to submit both forms simultaneously. Many bring the medical examination report to the interview.
